Producing sperm, fructose test ok, no sperm in ejactulate

I am at a loss. I show zero sperm in all ejaculate tests done. Fructose test shows ok(no blockage). Biopsy shows normal sperm production on both sides. How can this be?? First Urologist was stumped. Second Urologist from Northwestern University in Chicago reviewed all tests and slides from biopsy and ran another semen and fructose analysis and came to same conclusion that this is not normal and does not know why. He now wants to go in for exploratory surgery to poke around and see if he can see anything. I do not have insurance that covers this and the expense of this is killing me. He estimates this procedure will run about another $10,000.

Has anyone ever run into this?? I wouldn't mind paying if this was a means to an end but just to poke around and hope to see something that his own logic is saying it should not be...

Everywhere I look online too all I see is a blockage problem, or a production problem. What if it is neither and no(absolutely zero) sperm is in any ejaculate tests????

PS- we have considered IVF but my wife is over 40 so the chances of success are approx only 10%. This coupled with the expense for IVF make that a very very hard decision to make and one we have put way to the back for now.
