I am a 59 year old African-American male. I have sharp pain to the touch on my right nipple for the past 3 months. When anything rubs against it or while laying on it. I have noticed lately I have pain to the touch under my arm. What should be looked at?

You should see a physician. Chances are that you have a benign condition (probably inflammatory) that is causing your problem. Males do on very rare occasions get breast cancer. Very painful and tender problems usually do not indicate cancer, but some other problem that is benign.
They may want to do a mammogram or an ultrasound.
