PID, SEPTIC SHOCK, LAPAROTOMYHi, this is my first time in this [or any] forum but my recent health issues have left me with a lot of unanswered questions... Here's my story:
I'm a 23 y/o female with no history of major medical conditions other than a 4-day hospital stay due to very high levels of creatinine being produced by my kidneys. That was back in october. I've been in a monogamous relationship for the past 9 months. I have been diagnosed with BV a few times over the course of several years, but it's been several years since the last diagnosis. This past April I started experiencing flu-like symptoms-chills, body aches, and a fever that came and went for about a week. My fever went away with OTC medicine for the first 3-4 days, then was continuous for another 3-4 and stayed within a range of about 101.6-102.5 degrees. I went to my local walk-in clinic on Tuesday 4/14; they couldn't find a "diagnosable cause" so they gave me a general antibiotic and sent me home. That night I started developing severe lower [left side] abdominal pain and by 10AM the pain was EXTREME, so unbearable I had my mom come take me to the ER. PID was initially suspected but it took several hours to rule out all other causes. I had to endure a pelvic ultrasound-which was by far the most painful experience of my life- and numerous other tests (CT, uranalysis, x-ray, blood work etc.) PID was not officially diagnosed at this point because test results were not back yet. I was admitted into the hospital, still in extreme pain, and at 330AM I was taken to ICU with a fever of 104.7 degrees and BP that was dipping as low as 45/16. Still, no PID diagnosis was given although I was told I probably had developed septic shock syndrome. By 830AM I was in emergency surgery where an exploratory laparotomy and appendectomy were performed. About a liter of pus was removed from my lower abdomen and afterwards the official diagnosis of PID was given. All tests, including chladmydia and gonherrea, syphillis (*sp), HIV etc. came back negative. A culture of the infected tissue revealed that the bacteria to blame was alpha-hemolytic streptococcus. I was in the hospital for another 8 days and pumped full of antibiotics to get rid of the infection. Fast forward- last week I noticed an increased amount of discharge, odor... tell-tale symptoms of BV. I went to my OB today and sure enough, I have BV. Here's my issues: I was never given a clear answer as to how the streptococcus bacteria ended up in my reproductive tract. I realize it's often difficult to pinpoint one single cause but the ONLY possibility that the doctors & surgeons at the hospital offered was a contamination from my Nuva Ring. My OB found this highly unlikely but didn't offer any other possibilities as to how this bacteria, which is found virtually everywhere, decided to get comfy in my pelvis. Why weren't the natural acids and bacterium of my body able to fight it off? I'm still having abdominal pains but I'm not sure if it's just from my surgery healing or something worse... I do take my temperature pretty regularly and so far it's been within normal range but I still feel like that's not enough; like an infection can spring up at anytime and my temp'd still be a 98.6 lol. Furthermore, I'm understandably a bit more paranoid-so to say- about bacteria, germs etc. Yet, I'm not even 6-weeks out of major surgery and ALREADY another cluster of bacteria have managed to reproduce themselves enough to cause an infection?? Am I simply more susceptible to infections? If so, why? Could it be an overall immunodeficiency? I was told to "take it easy" after I was discharged from the hospital and my doc released me surprisingly quickly to go back to work & school. Using his "if it hurts-don't do it" advice, my boyfriend and I did start having sex again a couple weeks after the surgery. I won't lie-when it hurt, we stopped but I did probably over-do it, not just with that but resuming my everyday activities like taking care of a 4-year old, laundry etc. I'm terrified to go through what I went through again but it seems that my initial symptoms came so suddenly that I'm not even sure what I should be looking for! My partner and I have been tested, both before and after this whole incident, and no STD/STIs are or ever have been present. The information I've found shows that there's not a lot of evidence, or at least not conclusive evidence, that BV is directly linked to PID But now I have BV (which I haven't had in years) shortly after almost dying from a bacterial infection of a different kind. This worries [OK terrifies] me. I'm even having trouble finding cases where PID was so severe that it caused septic shock and required emergency laparotomy. I realize that because of the severity of my case, the chances of me ever conceiving naturally are slim to none... That breaks my heart. I have a 4-year old, wonderful son, but I'm only 23... Maybe I'm asking the one question that no doctor can answer, "Why me?" but if there's anything anyone knows or any advice on how I can avoid this in the future, I'd really appreciate it. I feel like my hospital stay back in october, which was ultimately attributed to a UTI in which I was asymptomatic until it had spread to my kidneys, and this episode of PID are somehow related but I just don't know how to connect the dots...
